LOCATION:

  • Brooklyn: 84 Linden BLVD, Brooklyn NY 11226 (Between Bedford and Rogers). Parking is limited, please leave time to find parking and/or consider taking public transportation.

  • Rockville Center: 165 North Village Ave, Suite 151, Rockville Center, 11570 (Use 30 Hempstead Ave entrance for easier access to suite)

IN-OFFICE SAFETY PRECAUTIONS

  • All patients must wear a mask to enter the office

LATE POLICY:

  • There is a 10-minute grace period

  • Patients more than 10 minutes late, WILL BE RESCHEDULED

  • Please also do not come too early as this may also cause in-office overcrowding

ADDITIONAL GUESTS:

  • Only the person being seen can enter the office

  • Minors can be accompanied by one parent/ guardian

  • Please do not bring additional guests to your appointment.

GENERAL

  • Please only bring one handbag 

  • No food/ eating allowed in the office

  • No cell phones in the patient examination areas

APPOINTMENT DEPOSITS

  • $75 deposit is required to schedule the appointment.

PAYMENTS

CANCELLATION/ RESCHEDULE POLICY:

  • Regular visit 

    • If you are unable to make your REGULAR OFFICE visit (or biopsy visit) appointment, kindly contact the office (via Klara, email, or phone) to cancel your appointment on or before 2 (TWO) BUSINESS DAYS PRIOR TO YOUR APPOINTMENT.

    • If the appointment is not canceled or rescheduled within the above-mentioned time frame, a $75 rescheduling FEE will be charged.

    • Appointment Deposit: This $75 deposit fee is a rolling deposit and will be kept on file for possible missed appointment fees. It will be refunded if no further appointments are scheduled.

  • Elective Procedures

    • If you are unable to make a scheduled ELECTIVE PROCEDURE (chemical peel, laser, filler, botox, PRP, etc.), kindly contact the office (via Klara, email, or phone) to cancel your appointment within at least 3 (THREE) BUSINESS DAYS PRIOR TO YOUR APPOINTMENT. If the appointment is not canceled or rescheduled within the above mentioned time frame, the total procedure deposit amount will be applied to a cancellation fee and a new deposit will be required to book another appointment. If you did not pay a procedure deposit and the visit is not canceled as detailed above, a minimum $100 (or the deposit amount paid) rescheduling FEE will be charged.
