Policies and Procedures

LOCATION:

  • Brooklyn: 84 Linden BLVD, Brooklyn NY 11226 (Between Bedford and Rogers). Parking is limited, please leave time to find parking and/or consider taking public transportation.

  • Great Neck: CLOSED AS OF NOVEMBER 21st, 2020

IN-OFFICE SAFETY PRECAUTIONS

  • All patients must wear a mask to enter the office

LATE POLICY:

  • We will not be able to accommodate any late NEW patients.  

  • If you are late, the appointment will be canceled and a $50 late payment will be charged.

ADDITIONAL GUESTS:

  • Only the person being seen can enter the office

  • Minors can be accompanied by one parent/ guardian

  • Please do not bring additional guests to your appointment.

GENERAL

  • Please only bring one handbag 

  • No food/ eating allowed in the office

  • No cell phones in the patient examination areas

  • Direct-Care Dermatology Model

APPOINTMENT DEPOSITS

  • $50 deposit is required to schedule the appointment.

  • Make a payment: See "payment" section below

  • Procedure deposits may vary

PAYMENTS

CANCELLATION/ RESCHEDULE POLICY:

  • If you have to reschedule your appointment (via Klara, email, or phone, text), please do so on or before 2 business days of the appointment and we will be happy to schedule you for a more convenient time.

  • If we DO get a cancellation on or before TWO (2) BUSINESS DAYS of the appointment, your $50 deposit will be refunded.

  • If you reschedule within TWO (2) BUSINESS DAYS, the $50 deposit is non-fundable and WILL applied to a new appointment.

  • If we DO NOT get a confirmation on or before TWO (2) BUSINESS DAYS of the appointment, your appointment will be CANCELLED and your $50 deposit WILL  BE refunded.

  • If you CONFIRM your appointment and DO NOT show up for your appointment, the deposit will be applied to a no-show fee. In order to schedule another appointment, a new $50 deposit is required.

  • Appointment Deposit: This $50 deposit fee is a rolling deposit and will be kept on file for possible missed appointment fees. It will be refunded if no further appointments are scheduled.

NEW PATIENTS / PATIENTS WHO HAVE NOT BEEN SEEN IN 12 OR MORE MONTHS

  • Please scan or send via klara (secure patient portal) a valid state ID

  • Please scan or send via klara a current and valid insurance card (will be used for biopsies and labs)

  • All forms must be updated and filled out prior to the visit (please see "paperwork" section)

 

PAPERWORK

  • Pre-Appointment Paperwork

    • Please fill out any paperwork including new patient forms prior to your visit to minimize the use of shared iPad devices (Especially for all new patients).

      • You can fill out all of your medical history via EZderm / Sadio portal (you will get an email to set up the account)

      • We can also send this to you via Klara if you can not set up your Sadio profile

      • Please use the Ezderm / Sadio patient portal to fill out all requested information including history, preferred pharmacy, and demographic information.

    • Please note that if you do not fill out any required paperwork and or answer questions at least TWO (2) business days prior to your visit, your appointment will be rescheduled
